Counseling Office Forms

Course Request Change/Five Year Plan Revision Form - used after registration is complete until school starts. This is to change a requested course. This form will only be available after registration in Spring 2019. This form needs to be picked up in the counseling office.

Schedule Change/Five Year Plan Revision Form - used the first 10 days of school to request a schedule change. This form will only be available during the first 10 days of school. This form needs to be picked up from the Counseling Office.

Level Change Form - used within the first 5 weeks of school to move from one level to another. This form will only be available as long as we accept level changes. This form needs to be picked up in the counseling office.

Course Withdrawal Form- mandatory to drop a course after 20 days of school until the end of the semester.

Student Information Change Form- use this to let us know when your personal information has changed.

AMCHS Food Pantry Form - available to anyone facing food insecurity in their lives (students and staff).
